OpenSSH 7.6 released

This is primarily a bugfix release. It also contains substantial
internal refactoring.

Security

 * sftp-server(8): in read-only mode, sftp-server was incorrectly
   permitting creation of zero-length files. Reported by Michal
   Zalewski.
